1. Your dentist
Your care is led by Dr. Urvashi Tanwar, MDS (Conservative Dentistry & Endodontics), registered with the Karnataka State Dental Council (Reg. No. 41463 A). Implants, orthodontics and oral surgery are carried out by visiting specialists, whom Dr. Urvashi introduces to you.
2. Examination and records
I agree to a dental examination, which may include:
- Questions about my medical and dental history, medicines and allergies
- A check of my teeth, gums, mouth and bite
- X-rays where they are needed for diagnosis or planning
- Clinical photographs of my teeth, kept in my confidential record
3. Treatment is planned with me
I understand that treatment is recommended only after the examination. Dr. Urvashi will explain what she has found, the options (including doing nothing), their likely benefits and risks, and the costs in a written estimate. No treatment will be carried out without my agreement.
4. Procedures and risks
I understand that dental treatment, like all medical treatment, carries some risks, which will be explained before each procedure. These can include temporary discomfort, swelling or sensitivity, reactions to local anaesthesia, and the possibility that a treatment does not succeed or needs to be redone. For some procedures, such as root canals, extractions, implants or surgery, I may be asked to sign a separate consent form in the clinic.
5. My health information
I agree to tell the clinic about my medical history, medicines, allergies and pregnancy truthfully, and to tell the clinic of any changes. I understand that incomplete information can affect my safety.
6. Patients under 18
The clinic sees children aged 10 and above. For a patient under 18, a parent or legal guardian gives this consent, comes to appointments and agrees to treatment on the child's behalf.
7. Photographs
Clinical photographs are part of my record. They will be shown on the website or social media, or used for teaching, only if I give separate written permission, and I can refuse without it affecting my care.
8. Fees
I understand that fees are explained in a written estimate before treatment, and that the Cancellation & Refunds policy applies to bookings.
9. Privacy and records
My information is kept confidential and handled as described in the Privacy Policy.
10. My right to change my mind
I may ask questions at any time, ask for a second opinion, and withdraw my consent or stop treatment at any time. Dr. Urvashi will explain what stopping may mean for my teeth.
11. Emergencies
If I have severe swelling spreading to the eye or neck, difficulty breathing or swallowing, or a high fever, I should go to a hospital emergency department straight away.
12. How my consent is recorded
When I book online, I tick boxes to confirm that I have read this consent. The date and version (version 1.0) are saved with my booking. If this consent changes in an important way, I will be asked to agree again.
